By completing and submitting this form, I confirm that I have read, understood, and agreed to all the following terms of the Independent Contractor Agreement with Nurture & Shine Inc.

  1. Independent Contractor Status
    I acknowledge that I am not an employee of Nurture & Shine Inc. I am fully responsible for my own taxes, insurance, and legal obligations.
  2. Work Assignments & Attendance
    • I agree to arrive on time for all scheduled shifts and appointments.
    • If I cannot attend or will be late, I must notify Nurture & Shine Inc. at least 2 hours in advance.
    • Repeated lateness or failure to attend may result in penalties or termination of my contract.
  3. Professional Conduct & Punctuality
    I agree to maintain professional behavior, respect client households, and provide services diligently and safely.
  4. Confidentiality & Privacy
    • I will not disclose any client personal information, addresses, or private matters to third parties.
    • Breach of confidentiality may result in immediate termination and legal consequences.
  5. Documentation & Legal Responsibility
    • I confirm that all documents I provide (ID, certifications, background checks, etc.) are authentic and valid.
    • I accept full legal responsibility if I submit false or fraudulent documents.
  6. Liability & Insurance
    As an independent contractor, I am responsible for my own health, safety, and insurance while performing services. Nurture & Shine Inc. is not liable for accidents, injuries, or damages during my work.
  7. Penalties & Termination
    Misconduct, negligence, or poor performance may lead to penalties, withheld pay, or termination of cooperation.
  8. Accountability for Work & Documents
    I accept full responsibility for the accuracy of my submitted documents and the quality of services I provide.
  9. Liability for Misconduct or Damages
    If any damages, complaints, or proven violations occur, Nurture & Shine Inc. may deduct compensation directly from my unpaid wages or future payments.
  10. Final Agreement
    By signing below, I acknowledge that I have fully understood and agreed to all the above conditions.
